Cancelation & No Show Policy
Failure to keep my appointment or give proper notice for the time reserved may result in a fee, as this time could have been given to another patient in need. If a 4 hour notice is not given, a cancellation fee of $30 for a visit and $50 for procedures will apply. Mental Health will need a 24 hour cancellation notice or a fee of $50 will apply. Unpaid no show balances will be forwarded to an outside collection agency if unpaid for 60 days.
How to Schedule an Appointment
Call our office 801-265-2000, phone hours are Monday – Friday 8:15 am – 11:45 am and 1:15 pm - 4:45 pm. Wait for the language options to complete and press 1 and wait for a scheduling representative to answer your call.

Questions for Your Provider & Care Team
If you are an established patient and have a questions for your care team (medical assistant or provider), you can bypass the phone tree if you allow the language options to complete, then select the options for your care team. Call 801-265-2000 then:
o Yellow Team

Weapons Policy
To ensure the safety and security of all employees, visitors, and tenants, the following policy is in effect at all times: No firearms, weapons, or dangerous devices are permitted inside the building. This includes, but is not limited to, handguns, rifles, knives, stun guns, pepper spray, or any other item designed to cause harm. Anyone who is in possession of a firearm or other weapon must leave it secured in their vehicle in the parking garage before entering the building.
The building, property management, and ownership are not responsible for any loss, theft, or damage to any firearm or weapon left in the parking garage or in a vehicle. Individuals assume all risk for any items left in their vehicles.
Failure to comply with this policy may result in:
· Denial of entry
· Removal from the premises
· Disciplinary action (if applicable)
· Law enforcement involvement if necessary
This policy applies to all employees, tenants, contractors, vendors, and visitors without exception.
